Volunteer Application Process

The first step will be to coordinate with one of our departments to begin the application process. The application will require the following:

  1. Volunteer Service Letter
  2. HIPAA & Privacy Training (PRV800) Please note: The Confidentiality Statement attestation is now completed in this training.
  3. General Compliance Training  (PBC810).
  4. FERPA Training (PRV802) *Only required for those with access to student data
  5. Health Assessment Form
  6. Copy of government issued ID; i.e., Driver’s License (if Foreign National then Gator1 Card or passport).
  7. Work authorization documentation required for foreign nationals: i.e., forms I-94, I-20, DS-2019 and/or EAD (front & back).
  8. Volunteer Request to Access Restricted Information
  9. Authorization for Volunteer Service Treatment of Minors
  10. Background checks will be conducted on all volunteer applicants. Please screenshot a clear, basic background check through HireRight with the volunteer application.

Once a notification is received that a volunteer has been approved, the department  partner will need to enter a Person of Interest (POI) ePAF into MyUFL.  Below are the Forms, Instruction Guides, and Toolkits to assist you with this process:
